Job summary

Abby Care is seeking a Registered Nurse to join as an RN Case Manager in the Merrimack Valley/Lawrence area of Massachusetts. You will coordinate patient care, work with PCPs and therapy teams, and ensure high-quality nursing services across a diverse caseload.

The role requires an MA RN license, 2 years of RN experience (1 year home health preferred), case management experience, and willingness to travel and be on-call. We offer strong benefits and growth opportunities.

Qualifications

• Active MA RN license.
• 2 years RN experience; 1 year home health preferred.
• Case management/supervisor experience preferred.
• Knowledge of Medicaid paid family caregiving program preferred.
• Reliable transportation and valid driver’s license required.
• Ability to travel locally and provide on-call service.

Responsibilities

• Manage a diverse caseload and coordinate care across care teams.
• Provide coaching and oversight to Home Health Aides for quality care.
• Develop and implement individualized care plans based on assessments.
• Coordinate with PCPs, therapy providers, and internal teams.
• Educate patients and families about care plans and health needs.
