Job summary

Phoenix Virtual Staff is seeking an experienced RN/LVN Case Manager to coordinate patient care, assess needs, develop plans, and support patients throughout their healthcare journey.

You will assess medical, functional, safety, nutritional, and psychosocial needs; create care plans; monitor progress; coordinate referrals; educate patients and families; and support discharge planning while maintaining HIPAA-compliant records.

Qualifications

• Active PHRN or USRN license is required.
• California RN or LVN license is preferred but not required.
• Minimum 2+ years US healthcare experience.
• Minimum 2+ years acute care experience.
• Minimum 2+ years managed care experience.

Responsibilities

• Assess patients' medical, functional, safety, nutritional, and psychosocial needs.
• Develop and maintain individualized care plans.
• Monitor patient progress, hospitalizations, ER visits, and readmissions.
• Coordinate care with doctors, specialists, hospitals, patients, families, and healthcare providers.
• Coordinate referrals, authorizations, and community resources.
• Provide patient and family education.
• Support discharge planning and continuity of care.
• Manage high-risk, complex, high-cost, and high-utilization cases.
• Participate in Interdisciplinary Care Team (ICT) meetings.
• Maintain accurate and confidential patient records in compliance with HIPAA.

Requirements;
