Job summary

Activate Care seeks an RN Manager to lead a distributed team of RN Supervisors and Home Health Assessors conducting in-person assessments for dually eligible members. This leadership role focuses on building supervision capability, standardizing clinical practices, and ensuring timeliness and quality across the program.

The role requires managing field-based teams, ensuring HIPAA/privacy compliance, and coordinating handoffs to care management. Travel and hybrid scheduling are expected.

Qualifications

• Active RN license in Massachusetts.
• BSN degree required.
• Minimum of 5 years RN experience including home health or community-based assessment.
• Minimum of 2 years supervising RN Supervisors or multi-team nursing operation.
• Proven ability to develop supervisors as leaders and coach the coach.
• Strong QA and audit program experience with data reporting.
• Experience with dual-eligible populations preferred.

Responsibilities

• Lead and develop RN Supervisors; set performance goals and growth plans.
• Provide coaching, performance reviews, and field observations.
• Manage bench strength and coverage plans during PTO/turnover.
• Set field-safety standards for in-home visits and remote nurses.
• Oversee QA audits of assessments and SOAP notes; ensure timeliness.
• Monitor KPI trends; drive corrective actions and training.
• Coordinate with health plan leadership and program teams.
• Address compliance, privacy, and HIPAA training across teams.
