Job summary

eData Services US, L.L.C. seeks a Remote Telehealth Registered Nurse to conduct nursing assessments, reassessments, monthly monitoring visits, and care coordination for ALW/CCT participants. This remote telehealth role uses telephone or video visits with required documentation completed promptly.

The RN will identify needs, safety concerns, and coordinate with families, facilities, and providers as needed. The ideal candidate holds a California RN license with at least 1 year of nursing

Qualifications

• Current and active Registered Nurse license in California.
• Minimum of 1 year of nursing experience.
• Experience with telehealth visits preferred.
• Knowledge of ALW, CCT, HCBS, Medi-Cal waiver programs, or care coordination preferred.
• Strong nursing assessment and clinical judgment skills.
• Strong documentation, communication, and follow-up skills.
• Ability to work independently in a remote setting.
• Proficiency with electronic medical records, email, phone communication, and telehealth platforms.
• Knowledge of HIPAA and patient confidentiality.
• Willing to work onsite at Mckinley, Taguig and US hours (Night shift).

Responsibilities

• Conduct initial nursing assessments for ALW and/or CCT participants via telehealth.
• Complete reassessments by telehealth to evaluate health status and safety needs.
• Assess medical history, medications, ADLs, mobility, cognition, fall risk, nutrition, psychosocial needs, caregiver support, and environmental safety.
• Ask follow-up questions to identify changes in condition or unmet needs.
• Escalate when telehealth is insufficient to in-person follow-up or urgent care.
• Document findings clearly and timely.
• Complete monthly telehealth visits and monitor changes in condition.
• Coordinate with participants, families, facilities, and healthcare providers.
• Provide health education within RN scope and document actions.
• Ensure privacy and confidentiality while working remotely.
• Maintain compliance with HIPAA and agency policies.
• Notify supervisor of technical issues or barriers to visits.
• Maintain secure records and use agency-approved systems.
