Job summary

eData Services US, L.L.C. is seeking a Remote Telehealth Registered Nurse to perform nursing assessments, reassessments, and monthly monitoring for ALW/CCT participants. Visits are conducted via telephone or telehealth platforms, with documentation completed accurately and timely.

The RN must work independently in a remote setting, provide care coordination, identify safety concerns, and escalate when in-person follow-up is needed. California licensure or active eligibility is required.

Qualifications

• Current and active RN license in California (or ability to obtain)
• Minimum 1 year nursing experience required
• Experience with older adults, assisted living, long-term care, home health or similar care settings preferred

Responsibilities

• Conduct initial nursing assessments for ALW/CCT participants via telehealth
• Perform monthly telehealth visits and monitoring of participant status
• Coordinate with participants, families, facilities, and care teams
• Complete nursing documentation accurately and timely
• Escalate urgent clinical concerns appropriately
• Maintain privacy and HIPAA compliance while working remotely
