The RN will conduct assessments, reassessments, and monthly monitoring visits via telephone or audio-video platforms, documenting findings promptly and coordinating with families, facilities, and care teams. The role requires strong clinical judgment, the ability to work independently, and strict adherence to HIPAA and program guidelines, ensuring high-quality patient care
