eData Services US, L.L.C. is seeking a Remote Telehealth Registered Nurse to manage ALW/CCT program assessments, reassessments, and monthly monitoring for participants. This is a remote telehealth role requiring independent work and strict documentation.

The RN will conduct visits by phone or video, coordinate with families and providers, ensure HIPAA-compliant records, and elevate safety concerns as needed while aligning with US hours.

Formación

• Current and active Registered Nurse license in the State of California for at least 1 year.
• Minimum of 1 year of nursing experience required.
• Experience with older adults, individuals with disabilities, assisted living, long-term care, home health, hospice, case management, or community-based programs preferred.
• Experience conducting telehealth visits preferred.

Responsabilidades

• Conduct telehealth nursing assessments and reassessments for ALW/CCT participants via telephone or video.
• Complete monthly telehealth visits and monitor participant status and safety.
• Coordinate care with families, caregivers, facilities, physicians, home health agencies, and community providers.
• Ensure timely and accurate documentation compliant with HIPAA and program requirements.
• Escalate urgent clinical concerns and maintain confidentiality while working remotely.
