Qualifications

• Profile 2 –Associate or college graduate of any course (except BS Nursing)
• With at least 2 years of direct patient care experience OR BPO healthcare voice account handling

Responsibilities

• Patient Assessment & Education: Conduct assessments, triage health concerns, and educate patients on managing chronic conditions (e.g., diabetes, hypertension)
• Care Coordination: Act as a liaison between patients and healthcare providers, ensuring follow-up on doctor recommendations
• Health Coaching: Guide patients on lifestyle modifications to improve health outcomes
• Documentation: Maintain accurate records of patient consultations and progress
