Qualifications

• Direct, hands-on experience using EHR/EMR systems to access and review patient records.
• Healthcare experience involving medical records, with familiarity with medical terminology.
• Understanding of how patient charts are organized and where to find clinical notes, histories, and procedure information.
• Proven attention to detail, including identifying conflicting information across multiple records.

Responsibilities

• Review assigned patient charts against study-specific inclusion and exclusion criteria.
• Examine clinical notes, medical histories, prior procedures, and other supporting records.
• Follow established checklists and standard operating procedures consistently.
• Cross-check information across the chart to identify inconsistencies or details that may affect eligibility.
• Document prescreening outcomes, reasons for exclusion, and other required information in our tracking spreadsheet.
• Flag missing information, conflicting records, and cases needing clarification from our team.
• Handle patient information confidentially and follow data-access and security procedures.
