Job summary

Outsourced.ph is seeking a Care Planning & Quality Assurance Officer to develop care plans from scratch, perform comprehensive risk assessments, and maintain accurate records using the Access system. The role involves structured QA calls with service users and close collaboration with the Registered Manager to ensure regulatory compliance.

The ideal candidate will have a nursing qualification or related health field, strong documentation and reporting skills, and experience with electronic care

Qualifications

• Registered Nurse (RGN, RMN, or RNLD) or degree in Health & Social Care, Social Work, Public Health, or related field.
• Evidence of ongoing professional development in health or social care.

Responsibilities

• Develop comprehensive, person-centered care plans based on initial assessments and referrals.
• Conduct detailed risk assessments (falls, moving and handling, nutrition, skin integrity, environmental, behavioral).
• Maintain 100% accurate electronic records within the Access care management system.
• Conduct structured quality assurance calls with service users and document feedback.
• Produce weekly reports for the Registered Manager and support regulatory inspections.
