Key responsibilities
Conduct psychosocial assessments and prepare case reports.
Develop and implement individualized care and intervention plans.
Provide counseling, case management, and support to residents and their families.
Facilitate individual, family, and group sessions as needed.
Coordinate with healthcare professionals, community partners, and government agencies.
Maintain accurate documentation and ensure compliance with professional and ethical standards.
Advocate for the well-being and successful reintegration of residents.
Qualifications
Registered Social Worker (PRC Licensed).
Strong interpersonal, communication, and case management skills.
Compassionate, empathetic, and committed to client-centered care.
Able to work effectively in a multidisciplinary team.
Experience in rehabilitation, mental health, or community-based programs is an advantage but not required.
