Job summary

ST CAMILLUS POLYCLINIC CHARITY FDN CAMILLIAN FATHER is seeking a Physical Therapist for resident care in Marikina. The role covers diagnostic tests, interventions, home instruction, caregiver guidance, ROM exercises, and treatment plan revisions in coordination with a Physiatrist.

The position involves scheduling residents, documenting evaluations and progress notes, and maintaining equipment. Fresh graduates are welcome; monthly salary ranges from Php 19,000 to Php 20,000.

Qualifications

• PRC license and eligibility to practice as PT required.
• At least 6 months experience or fresh graduates welcome to apply.

Responsibilities

• Performs all PT diagnostic tests and functional assessment for resident integration.
• Attends and performs direct PT interventions to residents.
• Provides home PT instruction, transfer techniques and self-care activities per physiatrist prescription.
• Instructs caregivers on bed and wheelchair positioning and transfer techniques.
• Provides ROM exercises for bedridden patients.
• Monitors patient progress and revises treatments in coordination with the Physiatrist.
• Notifies the nurse on duty for outpatient consultations.
• Schedules resident consultations with the Physiatrist and treatment programs.
• Conducts initial evaluations and prepares progress notes for follow-ups.
• Prepares rehab plans and endorsement notes with the Physiatrist.
• Monitors and updates resident charts and rehab inventory.
• Directs rehab concerns to the Physical Therapist Supervisor as needed.
• Monitors PT equipment and coordinates maintenance; prepares job orders and requisitions.
• Communicates with supervisors regarding referrals and coordination with departments.
• Participates in daily exercises with the event coordinator.
• Job types: Full-time, Fresh graduate; Pays monthly.
• Provides 13th month salary and OT pay as supplemental.

JOB DESCRIPTION:

A. PT Interventions for Residents

1. Performs all the PT Diagnostic tests, functional assessment scale for patient integration.

2. Attend and performs direct PT interventions to residents accordingly.

3. Provides home PT instruction, transfer techniques and self-care activities in accordance with prescription of the Physiatrist in-charged.

4. Instructs caregivers for proper bed positioning, wheelchair positioning and transfer techniques for the residents.

5. Provides ROM (Range of Motion) exercises for bedridden patients.

6. Protects the patient’s present status & progress or any problem encountered during treatment.

7. Revises the PT treatment program of each patient as per instructions of Physiatrist.

8. Notifies the Nurse on duty for outpatient consultation of residents.

B. Scheduling of patients

1. Schedules all resident’s consultation with the physiatrist retained by the Institution.

2. Schedules the treatment Program of all the residents.

C. Documentation and Reports

1. Conducts Initial evaluation prior to patient’s treatment

2. Prepares progress notes prior to patient’s follow-up consultation.

3. Prepares endorsement note for proper endorsement of the patient.

4. Draws up an individual rehabilitation plan of all PT clients with instructions and close coordination with the Physiatrist retained by the Institution.

5. Monitors patient’s chart every month.

6. Review resident’s chart after the Physiatrist consultation

1. Records all PT treatment and consultation in a logbook.

2. Directly approaches the Physical Therapist Supervisor with regards to important rehab concerns to coordinate it with the Geriatric Head.

D. Monitoring of PT equipment and supplies

1. Periodically monitors the condition of PT equipment

2. Recommends appropriate measures for maintenance of rehab equipment to

3. Administration

4. Prepares job orders for defective equipment and submits to the Maintenance Dept.

5. Prepares requisition forms for rehab supplies (tissue rolls, tape, office supplies, etc.)

6. Updates rehab inventory

E. Communication

1. Communicate with Physical Therapist Supervisor regarding referrals and other concerns to coordinate it with the other Department Heads.

F: Events and activities

2. Conducts exercises together with the event coordinator.

Participate on Daily morning and afternoon exercises of the residence together with the event coordinator.

Pay: Php19,000.00 - Php20,000.00 per month
