Job summary

This role administers the restoration of physical injuries, prevention of permanent disabilities, and improvement of fitness and mobility. The focus is on patient-centered care, guiding exercise routines, and monitoring progress to ensure optimal rehabilitation outcomes.

The successful candidate will develop individualized treatment plans, educate patients on home care, and maintain accurate records, while collaborating with the care team to support recovery goals and overall mobility

Responsibilities

• Explains treatment plans and performs Physical Therapy procedures to patients
• Facilitates patients’ responsibility for self-care
• Develops a treatment plan by selecting appropriate interventions
• Teaches patients how to properly use and execute therapeutic exercise techniques
• Maintains patients’ records and keeps track of goals and progress
• Monitors inventory and ensures availability of related medical supplies
• Submits census report every month
• Performs other tasks that may be assigned from time to time
