About the role

We are seeking a dedicated and knowledgeable Pharmacist to join the team at 1421 Medical Services/ Botica sa Tipas. This is a full-time position based in Ligid-Tipas, Taguig City, Metro Manila. As a Pharmacist, you will play a vital role in our healthcare delivery, ensuring that patients receive safe, effective, and appropriate pharmaceutical care. This position is strategically important to our organisation, as it directly impacts patient safety, medication management, and the overall quality of care we provide to our community.

Key responsibilities

• Be familiar with Philhealth/ YAKAP and Gamot Programmes; Will work in a small community-based pharmacy setting and will coordinate with the barangay health facility

• Accurately dispense medications according to prescriptions and ensure proper labelling with clear instructions for use

• Conduct comprehensive medication reviews to identify potential drug interactions, contraindications, and adverse effects

• Provide patient counselling on medication usage, side effects, storage requirements, and adherence to prescribed regimens

• Maintain accurate and complete pharmaceutical records and patient medication histories

• Manage pharmacy inventory, including ordering, receiving, and stock rotation of medications and medical supplies

• Ensure compliance with pharmaceutical regulations, licensing requirements, and professional standards of practice

• Collaborate with healthcare practitioners, including physicians and nurses, to optimise patient therapeutic outcomes

• Monitor and report adverse drug reactions and medication errors in accordance with established protocols

• Educate patients and healthcare staff on proper medication use and pharmacy services

• Maintain a clean, safe, and organised environment that meets hygiene and safety standards

What we're looking for

• Valid PRC ID of current pharmacist licence and registration with the relevant regulatory body

• Be willing to undergo orientation and training on the specific procedures and requirements of the GAMOT/Yakap program.

• Strong knowledge of pharmaceutical sciences, drug interactions, and therapeutic applications

• Excellent communication skills with the ability to explain complex pharmaceutical information clearly to patients and healthcare professionals; Ability to deal respectfully and patiently with Barangay residents, senior citizens, and other beneficiaries.

• Attention to detail and strong organisational abilities to manage multiple tasks accurately

• Knowledge of pharmaceutical regulations and compliance requirements in the Philippines

• Ability to work effectively in a fast-paced healthcare environment and handle patient interactions with professionalism and empathy

• Problem-solving skills and the ability to make sound clinical judgements
