Omega Healthcare Management Services, Inc. in Pasig, Philippines, seeks a US-licensed RN for on-site health & wellness support. You will triage patient inquiries, educate on chronic conditions, and document in Epic MyChart and Epic EHR, working the night shift.

Requirements include BS Nursing with active Philippines RN, US RN license (NY), and at least 2 years of clinical experience. Strong communication and computer skills are essential for patient care.

Competenze

• Must be a US Registered Nurse – New York Licensed.
• Graduate of BS Nursing with active Philippines Registered Nurse.
• With Clinical or Hospital experience of at least 2 years.
• Amenable to work on night shift.
• Ability to demonstrate critical and independent thinking.
• Comfortable with providing patient education related to chronic conditions such as diabetes, hypertension, asthma, COPD and heart failure.
• Excellent communication skills – written, interpersonal and via telephone/electronic.
• Strong computer skills including Microsoft Office suite and Electronic Medical Records.

Mansioni

• Assist Patients in prescription, visit follow up, non-urgent medical questions and test results.
• Review the in-basket continuously throughout the day to triage unread messages.
• Reach out to patient by telephone for urgent issues and with the local site via telephone or email as appropriate.
• Perform chart review and respond directly to patient through Epic MyChart.
• Route messages requiring clinical assistance from the site to the appropriate pool or provider.
• Process medication refill requests and route to provider.
• Document appropriately in the Epic EHR through the MyChart encounter.
• Review and complete if appropriate or route to local site administrative pool.
• Document appropriately in Epic EHR.
