Qualifications

Responsibilities

• Review patient Plan of Care for completeness, accuracy and alignment with protocols.
• Review Admissions Plan of Care & OASIS; code Home Health services.
• Recertify plan of care per client guidelines on home health services.
• Resume care and one-time visits per plan of care and client procedures.
• Act as clinical liaison between nurses, clinicians, case managers and physicians.
