Tan Tock Seng Hospital is seeking a Program Coordinator to support the Stroke Rehabilitation Ecosystem in coordinating, implementing and evaluating ecosystem activities under the leadership of the Program Clinical Leads. The role involves reviewing admissions, tracking patient progress, and ensuring effective referrals and partnerships.

Education in Nursing, Medical Social Work, Counselling, Psychology or Ethics is preferred, with independent initiative and strong communication skills essential

Qualifications

• Diploma or degree required.
• Background in Nursing, Medical Social Work, Counselling, Psychology or Ethics preferred.
• Experience in acute/intermediate and long-term healthcare advantageous.

Responsibilities

• Review daily admissions to identify and engage patients eligible for the program.
• Collate and track patient progress data and assist with analysis.
• Provide basic psychological interventions.
• Assist to redirect patients to suitable service tiers as needed.
• Maintain relationships with key agencies and community partners.
• Implement workflows for seamless referral and operations.
• Assist in planning and management of activities contributing to service quality.
