Overview
We are seeking a highly organized Home Health PT Coordinator & Documentation Specialist to support therapy documentation, visit coordination, physician orders, and related administrative workflows.
This role will be responsible for ensuring PT, OT, and ST documentation is accurately transferred into Axxess, visit frequencies remain aligned with the Plan of Care, and outstanding documentation and physician orders are tracked through completion. The role will also provide support with therapy utilization tracking, LUPA monitoring, invoice review, and administrative documentation workflows.
The ideal candidate is fast, accurate, proactive, and comfortable managing repetitive, deadline-driven processes independently. This is primarily a coordination and documentation role and does not require a clinical license.
Key Responsibilities
Therapy Documentation & Coordination
- Download PT, OT, and ST visit notes from the agency's therapy documentation platform and upload them into Axxess daily.
- Maintain accurate and timely therapy documentation within the patient record.
- Plot PT, OT, and ST visit schedules and frequencies in Axxess based on each patient's Plan of Care.
- Track actual therapy visit frequency against ordered frequency by discipline.
- Identify missed, late, incomplete, or inconsistent therapy documentation and elevate concerns appropriately.
- Maintain organized therapy documentation logs for ongoing tracking and audit readiness.
- Follow established workflows to ensure therapy documentation is uploaded and updated within required timelines.
Utilization & Visit Tracking
- Monitor therapy utilization and visit completion across assigned patients.
- Identify patients approaching Low Utilization Payment Adjustment (LUPA) thresholds and promptly notify the QA or clinical team.
- Track therapy visits against ordered frequencies to help prevent missed visits or documentation gaps.
- Compare scheduled, completed, and documented visits for consistency.
- Identify discrepancies in therapy visit tracking and elevate concerns to the appropriate supervisor.
- Maintain accurate utilization and visit tracking records for leadership visibility.
Physician Orders & Documentation Follow-Up
- Send physician orders and other required documentation for signature.
- Track outstanding physician orders through completion.
- Follow up on missing signatures, incomplete orders, and overdue documentation.
- Maintain accurate tracking of outstanding physician orders and documentation requirements.
- Ensure completed and signed documents are properly uploaded into the patient record.
- Escalate aging or unresolved items to the appropriate clinical or administrative leader.
- Provide consistent follow-up to help prevent delays in documentation completion.
Invoice & Administrative Support
- Review submitted invoices for accuracy and completeness.
- Enter approved invoice information into QuickBooks on a biweekly basis.
- Identify invoice discrepancies and communicate questions before processing.
- Maintain organized records supporting invoice review and entry.
- Assist with additional documentation, data entry, and administrative workflows as assigned.
- Support accurate and timely completion of recurring administrative tasks.
Cross-Functional & Operational Support
- Coordinate with therapy providers, clinical staff, QA, and administrative teams regarding documentation and visit tracking.
- Maintain organized trackers, logs, and other operational resources.
- Follow up on outstanding documentation and administrative items through completion.
- Help identify recurring documentation, coordination, or workflow issues.
- Escalate unresolved or time-sensitive concerns appropriately.
- Support additional therapy coordination, documentation, and administrative tasks based on business priorities.
Preferred Skills & Qualifications
- Prior Axxess experience strongly preferred.
- Strong experience with another U.S. home health EHR may be considered.
- Previous home health administrative, documentation, therapy coordination, or similar experience preferred.
- Strong understanding of visit frequencies and Plan of Care workflows.
- High level of accuracy when performing repetitive and deadline-driven data entry.
- Comfortable following structured workflows and maintaining detailed tracking logs.
- Strong organizational and time-management skills.
- Strong ability to proactively identify missing, incomplete, or inconsistent information.
- Excellent written and verbal English communication skills.
- Ability to work independently and consistently follow tasks through completion.
- Strong attention to detail and ability to manage multiple administrative priorities.
- Reliable remote work setup with stable internet connection.
- No clinical license required.
Nice-to-Have (Bonus Skills)
- Experience coordinating PT, OT, and ST services within a home health agency.
- Familiarity with LUPA thresholds and home health utilization tracking.
- Experience managing physician orders and outstanding signatures.
- QuickBooks experience.
- Experience supporting home health audit or survey-readiness activities.
- Experience with multiple home health EHR systems.
- Previous experience working remotely with a U.S.-based home health agency.
- Experience developing or maintaining documentation trackers, checklists, or workflow resources.
