Job summary

Ventra Health, Inc. is seeking a Chart Research Escalation Specialist to review medical records for proper indexing in EMR systems. The role serves as SME for the Chart Research Department and handles escalated issues from the team.

You will document processes, support training, assist with quality assessments, and coach teammates to ensure complete records with signatures and accurate ROS, HPI and MD notes across Cerner, Meditech, Epic, and PICIS environments.

Qualifications

• High School Diploma or GED.
• At least six (6) months of experience in handling patient health information and/or medical records.

Responsibilities

• SME for Chart Research Department; assists with team questions.
• - Handles escalated issues from team.
• - Documents work processes and supports training for new and tenured associates.
• - Assists with quality assessments.
• - Provides coaching to team members as needed.
• - Ensures medical records are complete within the EMR by obtaining missing chart information including physician signatures.
• - Reviews ROS, Exam, HPI, and MD Notes.
• - Retrieves existing medical records from EMR systems.
• - Scans and inputs documents; verifies proper indexing.
• - Pulls down medical records via Automate Tasks and monitors performance.
• - Logs into hospital systems and downloads registration logs and demographic data.
• - Creates new entries and sub-files as needed with correct medical record numbers.
• - Performs special projects and other duties as assigned.

Job Summary

• Chart Research Escalation Specialist is responsible for reviewing medical records for the necessary documents for proper indexing

Essential Functions And Tasks

• Serves as Subject Matter Expert (SME) for Chart Research Department. Assists with answering questions from team
• Handles escalated issues from team
• Documents work processes as required. Supports the training team with continuing education for new and tenured associates
• Assists with quality assessments
• Provides coaching to team members as needed
• Ensure medical records are complete within the electronic medical records system by identifying and obtaining any missing chart information including signatures from physicians and other pertinent personnel
• Conducts Review of Systems (ROS), Exam, HPI, and MD Notes
• Retrieves existing medical records from Electronic Medical Records (EMR) system
• Scans and inputs documents
• Verifies that information is indexed appropriately
• Pulls down medical records through Automate Tasks
• Monitors the performance of the Automate Tasks
• Logs into hospital systems and downloads registration logs and demographic data
• Create new entries and sub files as needed with correct medical record number
• Performs special projects and other duties as assigned

Education And Experience Requirements

• High School Diploma or GED
• At least six (6) months of experience in handing patient health information and/or medical records

Knowledge, Skills, And Abilities

• Working knowledge of medical terminology, anatomy and physiology, legal aspects of health information
• Working knowledge of department roles and responsibilities
• Knowledge of Electronic Medical Records (EMRs) such as Cerner, Meditech, Epic, PICIS
• Strong critical thinking skills
• Strong organizational skills
• Strong time management skills
• Strong word processing, spreadsheet, database, and presentation software skills
• Ability to read, understand, and apply state/federal laws, regulations, and policies.
• Ability to communicate with diverse personalities in a tactful, mature, and professional manner.
• Ability to remain flexible and work within a collaborative and fast paced environment.
• Basic use of computer, telephone, internet, copier, fax, and scanner.

Compensation

• Base Compensation will be based on various factors unique to each candidate including geographic location, skill set, experience, qualifications, and other job-related reasons .
• This position is also eligible for a discretionary incentiv e bon us in accordance with company policies .
