Job summary

Ventra Health is seeking a Chart Research Escalation Specialist to review medical records, ensure complete documentation in EMR systems, and support the team with accurate data indexing. The role involves extracting necessary chart information, ROS, HPI, and MD notes, while maintaining compliance with health information regulations.

You will work with EMR platforms (Cerner, Meditech, Epic, PICIS) and collaborate with the training team to improve processes and quality.

Qualifications

• High School Diploma or GED required.
• At least six months of experience handling patient health information or medical records.
• Experience with EMR systems such as Cerner, Meditech, Epic, PICIS.
• Strong analytical and communication skills.

Responsibilities

• Serve as SME for Chart Research Department and assist with questions from team.
• Handle escalated issues from team.
• Document work processes and support training for associates.
• Ensure medical records are complete in the EMR by obtaining missing information.
• Review ROS, Exam, HPI, and MD Notes.
• Retrieve existing medical records from the EMR system.
• Scan and input documents.
• Verify accurate indexing of information.
• Pull down medical records via automated tasks.
• Monitor the performance of automate tasks.
• Create new entries and sub-files as needed.
• Perform special projects as assigned.

Job Summary

• Chart Research Escalation Specialist is responsible for reviewing medical records for the necessary documents for proper indexing

Essential Functions and Tasks

• Serves as Subject Matter Expert (SME) for Chart Research Department. Assists with answering questions from team
• Handles escalated issues from team
• Documents work processes as required. Supports the training team with continuing education for new and tenured associates
• Assists with quality assessments
• Provides coaching to team members as needed
• Ensure medical records are complete within the electronic medical records system by identifying and obtaining any missing chart information including signatures from physicians and other pertinent personnel
• Conducts Review of Systems (ROS), Exam, HPI, and MD Notes
• Retrieves existing medical records from Electronic Medical Records (EMR) system
• Scans and inputs documents
• Verifies that information is indexed appropriately
• Pulls down medical records through Automate Tasks
• Monitors the performance of the Automate Tasks
• Logs into hospital systems and downloads registration logs and demographic data
• Create new entries and sub files as needed with correct medical record number
• Performs special projects and other duties as assigned

Education and Experience Requirements

• High School Diploma or GED
• At least six (6) months of experience in handing patient health information and/or medical records

Knowledge, Skills, and Abilities

• Working knowledge of medical terminology, anatomy and physiology, legal aspects of health information
• Working knowledge of department roles and responsibilities
• Knowledge of Electronic Medical Records (EMRs) such as Cerner, Meditech, Epic, PICIS
• Strong critical thinking skills
• Strong organizational skills
• Strong time management skills
• Strong word processing, spreadsheet, database, and presentation software skills
• Ability to read, understand, and apply state/federal laws, regulations, and policies.

Compensation

• Base Compensation will be based on various factors unique to each candidate including geographic location, skill set, experience, qualifications, and other job-related reasons.

• This position is also eligible for a discretionary incentive bonus in accordance with company policies.
