The Salvation Army is seeking a Registered Nurse Case Manager (Part-Time) to supervise and direct care for residents in a licensed Residential Care Facility for the Chronically Ill in Alegria. The RN will assess needs, coordinate services, and support medical plans under HIPAA and licensing requirements.

Responsibilities include administering medications, educating residents on health and life skills, and collaborating with the care team to ensure quality, compliant care.

Qualificações

• Current California license as a Registered Nurse (RN).
• 5 years of experience preferred.
• Must pass thorough live scan – criminal background check.
• Bilingual (English/Spanish) is preferred.

Responsabilidades

• Evaluate new residents with baseline physical and psychosocial assessments.
• Coordinate services per Individual Service Plan.
• Administer medications per physician order and manage PRN requests.
• Educate residents on health management and HIV/AIDS information.
• Attend monthly team meetings and update Service Plans.
