Job summary

ACS pro Global Corporation is seeking a qualified registered nurse (USRN) to manage the case load for quality home care patients in the Philippines. The role involves assessing patients across ages, coordinating with physicians, and documenting care plans and patient responses in CMS/DOH-compliant records.

The ideal candidate holds a New York USRN license, has EMR experience, and demonstrates strong communication, problem-solving, and organizational abilities.

Qualifications

• Active USRN license for New York state.
• Familiarity with EMR and Word/Excel preferred.
• Experience in Long Term and/or Acute Care Facility preferred.
• Strong communication, problem-solving, and organizational skills.

Responsibilities

• Assess and treat patients of all ages, including neonates, children, adults and geriatrics.
• Familiarity with MLTCP and commercial payer requirements for care.
• Address cognitive, physical, emotional and developmental needs across ages in treatment.
• Coordinate with physicians and other care providers to aid care planning.
• Maintain an ongoing caseload with accurate documentation.
• Document progress notes reflecting the plan of care and patient response.
• Participate in case conferences or clinical rounds to share guidance.
• Attend staff and team meetings as required.

Responsible for the case management of quality home care services rendered to patients.

Responsibilities

• Responsible for the assessment and treatment for patients of all ages, including neonates, child, adolescents, adults and geriatric.
• Familiarity with MLTCP and commercial payer requirements for care
• Understands the cognitive, physical, emotional and chronological processes for all ages. Treatment reflects an understanding of the developmental needs for each patient.
• Contact/follow up with physicians and other patient care providers when necessary to assist with care coordination.
• Maintains an ongoing responsibility for assigned caseload
• Documents accurately, timely and completely in patient’s clinical record in accordance with CMS/DOH regulations.
• Write an accurate and concise clinical/progress note that reflects implementation of the plan of care and the patient’s response to that plan of care.
• Participates in case conferences and/or clinical rounds to provide guidance ton care, while reinforcing best practices.
• Participate in staff/team meetings as required.

Minimum Qualifications

• Active USRN license for the state of New York
• Familiarity with EMR and computer applications (Word, Excel) preferred
• Experience in Long Term and/or Acute Care Facility preferred
• Strong communication, problem-solving, and organizational skills.
