Job summary

EasyHealth in the Philippines is seeking a Care Coordinator (CC), Clinical Team, who works directly with Nurse Practitioners (NPs) to provide intraday support and coordination to drive daily quality performance.

The CC partners with the NP team to support Chart Audit & Review (CAR) programs, perform clinical chart reviews, identify and close care gaps, track pending labs, and ensure documentation is complete and compliant with EasyHealth SOPs to sustain strong HEDIS/STARS outcomes.

Qualifications

• Active clinical background with US RN license and RN experience.

Responsibilities

• Organize NP team's daily workload and coordinate schedules.
• Track charts during review and notify NPs for finalization.
• Audit and review charts to verify care gaps addressed.
• Validate care gaps for measures such as osteoporosis, eye exams, A1C, CBP.
• Identify urgent issues and alert NP for timely member contact.
• Provide additional support to improve clinical quality.

Position Summary

The Care Coordinator (CC), Clinical Team works directly with a team of Nurse Practitioners (NPs) to provide intraday support and coordination that drives daily quality performance. The CC partners closely with the NP team to support the Chart Audit and Review (CAR) program, performing clinical chart review to confirm open care gaps are identified, addressed, and closed within visit, tracking charts pending lab results (waiting for labs), and ensuring documentation is complete and compliant with EasyHealth SOPs. This role is critical to maintaining clinical quality, closing care gaps in a timely manner, and supporting positive HEDIS/STARS outcomes.

Key Responsibilities

• NP Team Support: Organize and prioritize the NP team's daily workload, coordinate schedules, track outstanding tasks, and provide day-to-day operational support.
• Chart Management: Track charts throughout the review process, including charts waiting for lab results, and notify NPs when charts are ready for finalization and submission.
  
• Clinical Chart Audit & Review (CAR): Review charts after visits to verify that identified care gaps were properly addressed and documented according to company SOPs.
  
• Care Gap Validation: Check specific measures such as:
  
  • Osteoporosis management (OMW)
    
  • Eye/retinal exams (EED)
    
  • A1C testing and results
    
  • Controlled blood pressure (CBP) protocols
    
• Follow-up on Findings: When issues are identified—such as an A1C of 7.5 or higher—communicate with the NP so the member can receive appropriate education and follow-up guidance.
  
• Urgent/Emergent Issues: Identify urgent concerns from visits and immediately alert the NP, helping coordinate timely member contact when necessary.
  
• Additional Support: Perform other tasks that contribute to clinical quality, chart completion, and NP team performance.

Qualifications

• Active clinical background and US RN license with clinical RN experience and background.
• Strong attention to detail and comfort reviewing clinical documentation for completeness and accuracy.
• Ability to manage multiple charts/cases simultaneously and prioritize in a fast-paced, deadline-driven environment.
• Strong communication skills to coordinate effectively with Nurse Practitioners throughout the day.
• Comfortable working with EMR systems and internal tracking tools
