Job summary

Taranaki District Health Board is seeking experienced Allied Health Assistants or Health Care Assistants to join the START team, a new service delivering early supported discharge. You will work under registered professionals to support rehabilitation and enablement in patients’ homes, helping older people return home safely after acute events.

The role prioritizes teamwork, clear communication, and culturally responsive care with a focus on Te Tiriti o Waitangi and Kaupapa Māori principles.

Qualifications

• Proven experience as an Allied Health Assistant, Rehabilitation Assistant or Health Care Assistant.
• Excellent communication skills to effectively support patients, whānau, and staff.
• Strong relationship-building abilities.
• Passion for working within a high-performing team.
• Knowledge of Tikanga, Te Tiriti o Waitangi, and Kaupapa Māori principles, with a commitment to implementing Te Toi Ahorangi strategy.

Responsibilities

• Assist with the START early supported discharge model of care.
• Deliver rehabilitation and enablement in the home environment under delegation of registered health professionals.
• Support patients and whānau to achieve their health goals and participate in care planning.
• Assist with medication, nutrition, and carer stress management as part of the home-based care pathway.
