Job summary

Health New Zealand | Te Whatu Ora seeks an experienced mental health nurse for the Home Based Treatment Team in Hawke's Bay. You will deliver intensive, short-term care in patients' homes and in community settings, using advanced assessment and therapeutic communication to support recovery.

You will work from He Tāwharau – Crisis Hub, contribute to crisis response and care planning, and mentor learners while ensuring safe, culturally responsive practice across the locality.

Job description

Registered Nurse – Home Based Treatment Team, Hawkes Bay Fallen Soldiers Hospital. Permanent, Full-Time. 80 hours per fortnight (1 FTE). Make a meaningful impact by supporting tāngata whaiora in their own homes where recovery can be most effective.

About the Role

As a Registered Nurse within the Home Based Treatment Team (HBTT), you’ll provide intensive, short term mental health support to tāngata whaiora and their whānau in their own homes and community settings. Using advanced nursing assessment, therapeutic communication and evidence based practice, you’ll determine health needs, plan care, and deliver timely interventions that promote safety, autonomy and recovery.

Working from He Tāwharau – Crisis Hub, you’ll contribute to crisis response, acute care planning, mental health assessments, and home based treatment designed to prevent hospital admission where safe and appropriate. Accurate documentation, privacy, and culturally responsive practice will be central to your work. You may also take on portfolio responsibilities and support the learning of new staff and students.

About the Team/Service/Location

The Home Based Treatment Team is part of Te Whatu Ora’s community mental health services and operates from He Tāwharau – Crisis Hub in Hawkes Bay. The team provides an alternative to inpatient admission by delivering acute mental health care directly in the home environment. You’ll work closely with Clinical Coordinators, mental health assistants, nurses, allied health professionals, medical staff, and Māori and Pacific health partners across the locality. About You

You’re an experienced mental health nurse who brings compassion, resilience and excellent clinical judgement to fast paced, community based acute work. To succeed in this role, you will have:

Essential Qualifications

• A postgraduate qualification specific to mental health and addiction
• Current New Zealand Nursing Council registration and valid Advanced Practitioner Certificate (APC)
• Strong understanding and application of Te Tiriti o Waitangi in practice
• Effective time management and teamwork skills
• Full New Zealand driver licence
• Ability to engage therapeutically with tāngata whaiora and whānau
• Commitment to maintaining your Personal Development and Risk Management Plan (PDRP) and ongoing development

Desired Qualifications

• PDRP portfolio at a proficient or expert level
• Experience as a Duly Authorised Officer (DAO) or willingness to complete DAO training within six months

This role requires strong self awareness, reliability and the ability to remain calm and effective when working with people in distress.
