Job summary

Health New Zealand | Te Whatu Ora is seeking a registered Social Worker for the Community Child Health Team based at Waitakere Hospital. The role focuses on Tamariki and whānau within a health care lens, primarily community-based with some in-reach to Rangatira Ward.

You will work in a multi-disciplinary paediatric team, participate in an on-call roster across Child, Women and Family services, and travel to Waitematā sites as needed.

Qualifications

• APC registration with the Social Workers Registration Board (SWRB).
• Knowledge of child protection practices.
• Awareness of health, disability, and social services sector.
• Familiar with Tamariki/Rangatahi development and health needs.
• Understanding of whānau who have Tamariki/Rangatahi with disability or health needs.
• Full NZ driver’s license.

Responsibilities

• Provide social work support for Tamariki and whānau in the community.
• Work autonomously and in a multi-disciplinary team.
• Participate in the on-call roster for Child, Women and Family services on weekends/public holidays (8am–4:30pm).
• Travel to Waitemata sites as required.
• Contribute to strengths-based, whanau-centred care and ongoing professional development.

Social Worker Community Child Health0.6 FTE - Part TimeWe are looking for a registered Social Worker with a passion for working with, and supporting Tamariki and their whānau within a health care lens, to join our Community Child Health Team based at Waitakere Hospital.You will be joining an experienced and highly skilled multi-disciplinary paediatric community team who are committed to innovative best practice in the context of Tamariki-focussed/whānau centred care, that is based on the strengths and goals of the whānau. About the Role

As a Social Worker in the Community Child Health Team, you will be working in a multi-disciplinary environment, providing social work support for the whānau of Tamariki who are impacted by acute and/or long term health care needs. The role is primarily based in the community providing support for whānau in their home, with the addition of some in-reach social work support to the Rangatira Paediatric Ward.Following an orientation and settling in period, it is expected that you will participate in the Child, Women and Family On-Call roster providing support across our inpatient wards for child health, womens health and maternity wards. The on-call roster is active on weekends and public holidays from 8am-4.30pm.There may also be times where our Child Health Social Workers work across other Child, Women and Family areas, outside of the on-call roster, to offer support and service delivery across our division as needed - so a level of flexibility and adaptability would be required. You would receive training and support to manage this.In return we can offer a closely connected team and an environment that supports ongoing growth and career development. You will be provided regular supervision with the support of our Social Work Clinical Leader who works across both North and West Community Child Health Teams, as well as the Professional Clinical Leader for Social Work.The role is based primarily at Waitakere Hospital, but travel to other Waitematā sites (North Shore and West Auckland) will be required in this role.

Our Community Child Health Team is made up of dedicated allied health and nursing clinicians who share a passion for supporting Tamariki and their whānau who are faced with various health and disability challenges to live their best lives at home. Working in collaboration and in partnership with our whānau is central to our work, as is the drive to be strengths based. We have a hard-working team, that are closely connected - supporting each other on a daily basis, but who also enjoy bringing a sense of fun to their work environment. We maintain close working relationships across our inpatient paediatric areas and our community based Child Health and Womens Health services.

About you

Our ideal person will be passionate about providing care and improving health and social outcomes for Tamariki and their whānau. You will have proven ability to work autonomously as well as collaboratively in a supportive multi-disciplinary team environment.

Requirements

• A current APC and be registered with the Social Workers Registration Board (SWRB)
• Child Protection knowledge and experience
• Awareness of the health, disability and social services sector
• Be familiar with the developmental and/or health needs of Tamariki/Rangatahi
• Awareness of the support required by Whānau who have Tamariki/Rangatahi with a disability or a health need
• A full NZ drivers license
