Health New Zealand | Te Whatu Ora is seeking a part-time coordinator to perform Needs Assessments and Service Coordination for people with disabilities across the Kapiti and Wellington region. You will engage with clients and providers to arrange supports, ensuring person-centred and equitable care.
The role is based at Kenepuru Hospital, with collaboration across the Capital Support team. The position offers 0.75 FTE (30 hours/week), with hours negotiable and a focus on delivering enabling good

Contract:Permanent, Part Time (0.75 FTE / 30hours per week, Monday to Friday - Hours each day is negotiable)

Serving our typically under 65 years ofage disabled community to provide needs assessments wherever they chose to meetfrom South Wellington to Peka Peka in Kapiti and all in between. Essentiallyyou will be completing Needs Assessments and then Service Co-ordination forpeople with Intellectual, ASD, sensory and physical disability as per criteriafor Disability support services, but the role is much more encompassing thanthat. Relationships with contracted providers and many other key stakeholders.
