Job summary

Responsive Health Management Inc. is seeking a Registered Practical Nurse for a part-time, on-site role in Hamilton, Ontario. The RPN will coordinate resident-focused care, supervise staff, and ensure compliance with nursing standards and home policies.

The successful candidate must hold a current RN certificate and CPR, with experience in long-term care being an asset. Strong communication and leadership skills are essential.

Qualifications

• RN/Registered Nurse with applicable provincial license
• Basic CPR certification required
• Experience in long-term care is an asset
• Strong written and oral communication skills
• Ability to collaborate with interdisciplinary teams
• Knowledge of RAI‑MDS – AIS is preferred or to be completed within 1 year

Responsibilities

• Develop and modify care plans to meet residents' changing health needs
• Liaise with health care personnel, agencies and families
• Supervise unit activities and provide leadership to staff
• Monitor performance and evaluate PSW and RN staff
• Direct and evaluate work of PSWs
• Ensure care aligns with residents' rights and home policies
• Administer medications and treatments per policies
• Maintain accurate nursing records and reports
