Responsive Health Management Inc. is seeking a Registered Practical Nurse (RPN) for a regular part-time position in a long-term care setting.

The RPN will provide evidence-based, patient-centered nursing care, perform assessments, and coordinate with the care team to meet residents' goals while maintaining dignity and safety. Strong documentation and collaboration with interdisciplinary staff are essential.

Qualifications

• Current General Class registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse (RPN).
• Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse or equivalent outside Ontario, with right-to-practice in Ontario where applicable.
• Current CPR/BLS certification.
• Familiarity with interRAI assessment processes and care planning is an asset.

Responsabilités

• Provide safe, compassionate person-centered nursing care—upholding residents' dignity, choice and quality of life.
• Complete nursing assessments and collaborate with the RN/physicians to adjust care plans.
• Document accurately and maintain records per regulations and home policy.
• Administer medications and treatments safely following policies and procedures.
• Support IPAC practices and ensure infection prevention standards are followed.
• Collaborate with PSWs and interdisciplinary team to deliver high-quality care.
• Participate in audits, quality initiatives, and readiness for accreditation.
