Health New Zealand | Te Whatu Ora in Tauranga is recruiting a Needs Assessor/Service Coordinator to perform InterRAI Home Care assessments for older adults with complex needs, coordinating services with clients and whānau. Training in InterRAI will be provided; you’ll work in a supportive team across the Bay of Plenty to promote hauora at home and equity in care, with options for 0.9 or 1.0 FTE.

A compassionate, proactive clinician with a current Annual Practising Certificate and a full NZ

Qualifications

  • Registered Health Professional with current Annual Practising Certificate.
  • Experience with older adults in community settings preferred.
  • Willingness to travel with valid NZ driver's licence.

Responsibilities

About the Role

The Needs Assessor/Service Coordinator role focuses on undertaking InterRAI Home Care assessments for older adults with complex needs, identifying both strengths and areas of support. Working closely with clients and their whānau, you will coordinate services that enhance quality of life and promote Hauora in the home setting. Training in the InterRAI system will be provided, but a strong level of digital confidence and adaptability is essential. This is a rewarding opportunity to work within a team that supports equity, compassion, and excellence in care.

We are looking to appoint:

  • One position at 0.9FTE (36 hours per week)
  • One position at 1.0 FTE (40 hours per week)

About the Team/Service/ Location Support Net

Support Net is part of the Regional Community team based in Tauranga. The Health of Older People team works in collaboration with kaumātua, whānau, community providers and other health professionals to ensure safe, effective care plans are in place. What you will bring
  • It is essential that you are a Registered Health Professional with a current Annual Practicing Certificate
  • A full NZ drivers licence and willingness to travel for client visits

  • Experience working with older adults and their whānau in a community setting
  • Understanding of InterRAI assessment processes (training provided)
  • Ability to quickly learn new systems
  • Working for Health New Zealand
