WellSouth Dunedin invites applications for an experienced Health Coach to support whaiora and whānau in managing health and wellbeing within a primary care setting. You will help clients progress lifestyle changes, including activity, sleep, and chronic condition self-management, and facilitate group activities.

You’ll work in a busy, client-focused team with initial training and a 3-month induction. We value flexibility, equity, and ongoing professional development to grow with the role.

Qualifications

• Experience delivering health coaching within primary care.
• Ability to support whaiora to set goals.
• Experience working with diverse patient groups of all ages.
• Willingness to participate in training and induction.

Responsabilités

Health Coach

Location: Dunedin

FTE: 1.0

We are seeking an experienced Health Coach who are passionate about supporting whaiora to manage their health and wellbeing. We are keen to hear from applicants interested in a career that supports people within their community who want to make lifestyle changes to better manage their health.

This diverse and fulfilling role empowers whaiora and whānau to gain the knowledge, skills, and self-confidence to reach their own health and wellbeing goals. This includes lifestyle changes (smoking cessation, exercise), promoting self-management (especially for managing long-term conditions like diabetes), facilitating groups and helping navigate primary care and the wider social services as needed for success.

About the role:

Health Coaches are a high-impact, accessible resource within general practice, supporting 8-10 whaiora a day. Empowering individuals, whānau and groups to set goals and make positive changes in areas such as:

• Encouraging good management of various long-term conditions
• Discussing how to be more active by doing things they enjoy
• Improving sleep habits
• Improving overall wellbeing

You will be well-supported in this role with initial training and a comprehensive 3-month induction to help integrate into practice. We value your growth and development as a Health Coach, providing additional professional development to support you in this role.
