Job summary

HealthCare of New Zealand in Rangiora is seeking an experienced Support Worker to assist a client at home and in the community, following a care plan to promote independence and wellbeing.

Roster includes Week 1: Sat/Sun 07:00–15:00; Week 2: Thu 15:00–23:00 and Sat/Sun 07:00–15:00. Candidates should have Level 3 or 4 healthcare qualifications and a NZ driver's licence with reliable transport.

• Experience in in-home or community-based care is preferred.
• Ability to follow care plans and document care activities.
• Good communication with clients and families.

Responsibilities

We are currently seeking an experienced Support Worker to provide support for a client based in Rangiora.

Roster:

Week 1

• Saturday and Sunday:07:00 AM – 03:00 PM

Week 2

• Thursday: 03:00 PM – 11:00 PM
• Saturday and Sunday:07:00 AM – 03:00 PM

What does a Support Worker do?

As part of a wider team, you will assist our clients to live as independently as possible, within their homes and community. You will help our clients to achieve their goals and reach their potential, through both practical and emotional support. Responsibilities can be varied and you will follow a care plan, which may include some or all of the below:

• Meal preparation and community outings and integration
• Giving medication
• Personal cares (showering, toileting etc)
• Home management
• Must follow care plan instructions
• Level 3 or 4 qualified
• Hoist trained- sara steady, Catheter cares (empty catheter 3 hourly or if full and night bag change)
• Familiarity with passive exercises
