Job summary

Ngāti Hine Health Trust’s Disability Support Services is seeking Home Support Community Workers to provide person-centred support to people living with disabilities in their homes and communities. You will help individuals to be independent, build confidence and participate in everyday activities.

You will work with tāngata whaikaha and whānau to enable goals and aspirations, with flexible hours including evenings and weekends as required.

Qualifications

• Experience in disability support, home support, healthcare or community services.
• First Aid certificate or willingness to obtain one.
• Health and Wellbeing Level 2 or willingness to work towards.
• Full NZ driver licence.

Responsibilities

• Assist with personal care and daily living activities.
• Support participation in community, social, recreational and cultural activities.
• Implement individual support and lifestyle plans.
• Support people to develop life skills and achieve personal goals.
• Encourage independence, choice and self-determination.
• Provide companionship and emotional support.
• Maintain accurate records and reporting requirements.
• Work collaboratively with whānau, colleagues and other services.
