Job summary

Ngāti Hine Health Trust invites applications for Home Support Community Worker in Whangārei. Provide person‑centred support to individuals living with disabilities at home and in the community, working with tāngata whaikaha and whānau to foster independence and confidence.

You will deliver personal care, assist with daily living, promote participation in cultural and community activities, and implement personalised support plans guided by kaupapa Māori values and Te Tiriti o Waitangi.

Qualifications

• Experience in disability support, home support, healthcare or community services.
• Current First Aid Certificate (or willingness to obtain).
• Full NZ Driver Licence.

Responsibilities

• Assist with personal care and daily living activities.
• Support participation in community, social, recreational and cultural activities.
• Implement individual support and lifestyle plans.
• Help clients develop life skills and achieve goals.
• Encourage independence, choice and self‑determination.
• Provide companionship and emotional support.
• Maintain accurate records and reporting requirements.
• Collaborate with whānau, colleagues and other services.
