Job summary

Health New Zealand | Te Whatu Ora is seeking a part-time coordinator to perform Needs Assessments and Service Coordination for people with disabilities across the Kapiti and Wellington region. You will engage with clients and providers to arrange supports, ensuring person-centred and equitable care.

The role is based at Kenepuru Hospital, with collaboration across the Capital Support team. The position offers 0.75 FTE (30 hours/week), with hours negotiable and a focus on delivering enabling good

Qualifications

• Experience within the disability sector is desirable but not compulsory.
• Excellent interpersonal skills and ability to build rapport with clients and stakeholders.
• Strong prioritisation and workload management.
• Proficiency with basic computer applications.
• Commitment to Enabling Good Lives principles and Te Tiriti o Waitangi.

Responsibilities

• Conduct Needs Assessments and Service Coordination for people with disabilities.
• Coordinate with contracted providers and key stakeholders to arrange services.
• Maintain client relationships and comprehensive documentation across caseloads.
