Job summary

CareChoice Pty is seeking a Disability Support Worker based in Branxholme to provide person-centred, in-home and community-based support to people with disability, promoting independence and quality of life.

Working as part of the casual Support Workers team, you will assist clients with personal care, mobility, domestic tasks and community access while following individual support plans and organisational policies.

Qualifications

• Minimum 2 years of experience in disability or related support work.
• Certificate III or IV in Disability, Individual Support, Aged Care or related.
• Experience providing personal care and mobility support.
• Experience using hoists and assistive equipment safely.
• Experience enabling community access and transport support.
• Understanding of person-centred practice and inclusion.

Responsibilities

• Provide person-centred in-home and community support.
• Assist with personal care, mobility, and domestic tasks.
• Support clients with community access and appointments.
• Follow individual support plans and health care plans.
• Document progress notes and reports accurately.
• Collaborate with families, carers, and professionals to achieve goals.
• Maintain confidentiality and professional boundaries.
• Engage in ongoing training and team meetings.
