Job summary

Health New Zealand | Te Whatu Ora in Greymouth is seeking a Home and Community Support Services Registered Nurse/Enrolled Nurse Assessor to assess health needs in community settings and develop tailored care plans using InterRAI.

You'll collaborate with clients, families and the healthcare team to achieve optimal outcomes, train healthcare assistants on ADLs, and support staff development with the HCSS Lead in a 0.8 FTE permanent role.

Qualifications

NZ Registered Nurse/Enrolled Nurse with current Annual Practising Certificate
Full NZ driver's license and good computer literacy
Ability to work autonomously within a team
Excellent communication and interpersonal skills with diverse groups
Commitment to ongoing professional development and confidentiality
Understanding of Treaty of Waitangi principles

Responsibilities

Conduct comprehensive clinical assessments using InterRAI to inform care planning
Develop individualized care plans and goals for clients receiving home and community care
Provide training and support for healthcare assistants on ADLs and home-based care
Collaborate with the HCSS Lead to support staff development and training
Respond promptly to community referrals (target within 48 hours)
Maintain high nursing standards in line with policies and code of rights
Participate in quality improvement and interdisciplinary team discussions
