Health New Zealand | Te Whatu Ora in Greymouth is seeking a Nurse Assessor for Home and Community Support. You will assess health consumers in community settings using InterRAI and develop tailored care plans.

You’ll train healthcare assistants on ADLs and collaborate with the broader care team to ensure quality outcomes. The role requires NZ Registered/Enrolled Nurse status, current APC, and strong communication.

Qualifications

  • NZ Registered Nurse or Enrolled Nurse with current APC.
  • Understanding of InterRAI guidelines and assessments.
  • Experience in community or home-based care settings.

Responsibilities

  • Conduct comprehensive clinical assessments using InterRAI to inform care planning.
  • Develop individualized care plans and goals for clients receiving home and community-based care.
  • Provide one-on-one training and support for healthcare assistants on ADLs and home-based care practices.
  • Collaborate with the HCSS Lead to support staff development and training initiatives.
  • Ensure timely responses to community referrals (within 48 hours where possible).
  • Maintain high standards of nursing care in line with NZ policies and the Code of Health & Disability Services Consumer Rights.

About the Role:
As a Home and Community Support Services (HCSS) Registered Nurse/Enrolled Nurse Assessor, you will use your clinicalexpertise to assess the needs of health consumers in community settings anddevelop tailored care plans using the InterRAI assessment tool. You’llcollaborate closely with clients, their families, and the wider healthcare teamto ensure the best possible outcomes for those in your care.

Key Responsibilities:

  • Conduct comprehensive clinical assessments usingInterRAI to inform care planning.
  • Develop individualized care plans and goals forclients receiving home and community-based care.
  • Provide one-on-one training and support forhealthcare assistants on activities of daily living (ADLs) and home-basedcare practices.
  • Collaborate with the HCSS Lead to support staffdevelopment and training initiatives.
  • Ensure timely responses to community referrals(within 48 hours where possible).
  • Maintain high standards of nursing care, aligningwith National and Health NZ policies and the Code of Health &Disability Services Consumer Rights.
  • Use a partnership approach to enhance healthoutcomes for health consumers.
  • Participate in quality improvement initiatives andinterdisciplinary team discussions.

About You

EssentialQualifications & Experience:

  • NZ Registered Nurse/Enrolled Nurse with a currentAnnual Practicing Certificate.
  • A full, clean NZ driver’s license.
  • Strong computer literacy and a willingness to learnnew systems.
  • Ability to work autonomously while contributing toa collaborative team environment.
  • Excellent communication and interpersonal skills —comfortable engaging with people from diverse backgrounds and cultures.
  • Cultural sensitivity and understanding of thePrinciples and Articles of the Treaty of Waitangi.
  • High level of self-presentation andprofessionalism.
  • Ability to maintain confidentiality and exercisediscretion.
  • Capacity to work under pressure and managecompeting priorities.
  • Commitment to ongoing self-development andprofessional growth.
  • Well-established professional boundaries andaccountability within the scope of nursing practice.
  • InterRAI competency and knowledge of InterRAIguidelines.
  • Previous experience in a community nursing setting.
  • Understanding of home and community support systemsand service delivery.
