Job summary

Enliven, part of Presbyterian Support Northern, is seeking a fixed term Service Coordinator to balance clinical practice with coordination and service improvement in home-based and community support for older people. You will work with service users, whānau, and partners to ensure timely, person-centred care and align with contractual requirements.

Qualifications

Must be a Registered Nurse, Enrolled Nurse, Occupational Therapist or Physical Therapist (essential).
Full New Zealand driver's license with no restrictions.
Experience in aged care, community health or rehabilitation.
Experience coordinating services and working with multidisciplinary teams.
Ability to manage competing priorities in a fast-paced environment.

Responsibilities

Coordinate services so users receive the right support at the right time.
Undertake clinical assessments and reviews for non-complex clients.
Develop and maintain individual support plans focused on independence and wellbeing.
Support rostering, scheduling and matching of staff and users.
Build relationships with NASC, DHBs, GPs and community partners.
Contribute to reporting, quality systems and service improvement initiatives.
Support staff development, engagement and a positive team culture.
Promote culturally appropriate practice aligned with Tangata Whenua values.
