Job summary

Healthvision is seeking a skilled Community Nurse to deliver high-quality nursing care in patients’ homes across the Bay of Plenty. You will manage a caseload with clinical autonomy, collaborate with GPs and other professionals, and support diverse service users and their whānau.

The role emphasises wound care, catheter management, medication support and education, with ongoing professional development and a wraparound in-home care model.

Qualifications

• Current NZ Nursing Council practising certificate.
• Full or Restricted New Zealand driver’s licence.
• Access to a reliable vehicle.
• Clean Police Clearance.
• Eligibility to legally work in New Zealand.
• Flexibility to travel throughout the Bay of Plenty.

Responsibilities

• Comprehensive nursing assessments and clinical reviews.
• Developing, implementing and evaluating individualised care plans.
• Providing wound management and catheter care.
• Medication management and other clinical interventions.
• Health education for service users and whānau.
• Support and educate Support Workers and collaborate with other health professionals.
