Job summary
Access Community Health is seeking a Community District Nurse to deliver essential nursing care in clients’ homes across a rotating roster in Christchurch. The role offers autonomy and strong clinical support from a multidisciplinary team, including palliative care specialists, support workers, and allied health professionals.
Candidates should be Registered or enrolled with a current practising certificate and have experience in wound care, medication administration, and chronic disease
Qualifications
- Must be Registered or enrolled with a current practising certificate.
- Experience in community or extended nursing roles is preferred.
- Ability to work autonomously with strong clinical judgement.
- Strong time management to handle busy caseload across home visits.
- Comfort with using healthcare apps and digital tools.
Responsibilities
- Deliver essential nursing care in clients' homes.
- Liaise with multidisciplinary team including palliative care, support workers, and allied health professionals.
- Manage medication administration, wound care, diabetes management, and catheter care.
- Provide palliative and end-of-life care; participate in on-call roster as required.
- Build therapeutic relationships with clients and whānau in the community.
