Job summary

Qualifications

Responsibilities

• Coordinate care by linking patients with proper care team member or resources.
• Schedule medical appointments and manage referrals.
• Handle faxes, mail, and multi-line phone communications.
• Identify patients for outreach and follow-up.
• Maintain documentation for all patient encounters.
• Support billing efforts as directed by leadership.
• Collaborate across teams to improve care processes.
