A.G. Rhodes Cobb community in Marietta, GA is seeking a Full-time Physical Therapist – Care Partner to deliver person-directed PT services to elders in a compassionate, team-oriented environment.

We offer a $5,000 sign-on bonus, robust benefits, and a supportive setting with memory care focus. One year of long-term care experience is preferred; Georgia PT license is required. This nonprofit emphasizes compassionate, inclusive care and strong retention.

Qualifications

• Current Georgia PT license in good standing.
• At least 1 year of experience in Long-term Care/Skilled Nursing.
• Experience in Person-Directed Care community preferred.
• Genuine passion for working with elders.
• Strong teamwork and communication skills.
• Good documentation skills (Net Health preferred).
• Strong organization and time management skills.

Responsibilities

• Provide Physical Therapy services to residents in memory care and skilled nursing.
• Collaborate with care partners to develop and implement care plans.
• Document assessments and progress notes accurately in electronic charting.
• Assist with discharge planning and rehabilitation goals.
• Support high-quality resident care and safety through evidence-based practices.

Physical Therapist Care Partner Qualifications Include

• Current Georgia Physical Therapist PT license in good standing
• One year of experience working in a Long‑term Care/Skilled Nursing community but willing to train new grads
• Experience working in a Person-Directed Care community preferred
• Genuine passion for working with elders
• Strong team work/collaboration skills
• Excellent customer service and communication skills
• Good documentation skills (electronic charting experience with Net Health preferred)
