Job summary

Citizens Options Unlimited is seeking a Direct Support Staff member to provide comprehensive in-home support and promote active participation in daily living. You will assist with chores, meals, medication, and health documentation, while following treatment plans and safety protocols.

The role requires AMAP training, basic first aid, and a willingnes s to work closely with nurses and supervisors to ensure wellbeing and progress for clients.

Qualifications

• High school diploma or equivalent preferred.
• Qualified NYS driver's license required and maintained.
• Ability to understand written and oral directives.
• AMAP Training is required within 6 months of hire.
• First Aid/CPR/SCIP-R Training is preferred.

Responsibilities

• Ensure a safe, clean home by assisting with household chores and participating as needed.
• Follow treatment plans and use adaptive/safety equipment to promote independence.
• Assist with meal preparation, including diet adherence and portion control.
• Assist with medication administration per AMAP policy and procedures.
• Work with residential nurses to maintain medical documentation.
• Implement and document treatment plans and related materials.

Overview

Practices and promotes the agency’s mission statement and promotes the agency’s guiding principles in every interaction. Provide ongoing support to people in becoming active participants in every aspect of their lives (i.e., self-care routines, health, safety, household chores, socialization, and overall wellbeing) and to ensure that a high quality of life is maintained.

Responsibilities

Primary duties and responsibilities include but not limited to:

• - Ensures a safe and clean home by providing support to people to assist them with the completion of household chores and by participating in said household chores as needed
• - Follows all treatment plans and utilizes all corresponding adaptive and safety equipment needed for each person to promote independence and ensure their health, safety and wellbeing.
• - Supports people in preparing meals by teaching concepts such as diet adherence, portion control and cooking skills
• - Assists people with medication administration according to the AMAP policy and procedures
• - Works with residential nurses to ensure accurate and current medical documentation for people supported if maintained.
• - Implements and documents all treatment plans and other agency related materials (i.e., residential habilitation goals, medical case notes, communication logs, etc.) to assist people in achieving independence in their selected valued outcomes and to ensure their health, safety and well-being
• - Adheres to overtime policy and procedures to ensure that quality support is provided to people each day
• - Attends all necessary meetings to discuss each person’s progress and goal achievement and offer and gather information and ideas
• - Provides transportation as needed for emergency situations
• - Mentors new Direct Support Staff by demonstrating supportive routines and by reviewing training manuals to ensure new employees are sufficiently groomed to complete responsibilities.
• - Responsible for documenting sleep checks through the night for the people supported
• - Responsible for knowing and participating in all emergency evacuation procedures
• - Performs all other job-related duties as requested by supervisor

This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check

Qualifications

• - High school diploma or equivalent preferred.
• - Qualified NYS drivers' license required. And maintained throughout employment.
• - Ability to communicate and understand written and oral directives/information.
• - AMAP Training is required and must be completed within six (6) months of hire.
• - First Aid/CPR/SCIP-R Training is preferred.
