CarePartners is seeking a Personal Support Manager to join our leadership team in Sault Ste Marie, Ontario. The role coordinates and supervises delivery of Community Health Services, mentors care teams, and drives quality patient care on a Monday–Friday schedule with on-call rotation evenings and weekends.

The ideal candidate is an RN or RPN with 3–5 years of leadership experience, registration with CNO, and strong communication skills. Travel and vehicle use are expected for community coverage.

Competenze

• Must hold RN or RPN registration with the College of Nurses of Ontario (CNO)
• 3–5 years of related leadership experience
• Experience in community health services is an asset
• Proven computer skills including email, word processing and scheduling systems
• Excellent English verbal and written communication skills
• Valid CPR certification and clear background check

Mansioni

• Provides coordination, supervision, and coaching to Community Support Workers
• Develops and maintains the Care Plan for Community Health Services
• Monitors and enforces Health and Safety policies
• Promotes and markets Community Health Services and maintains public relations
• Supports workplace Health and Safety initiatives
• Performs additional duties as required
