Job summary

NTUC Health Co-operative Limited’s CREST team is seeking a Case Associate to support outreach initiatives and monitor the wellbeing of clients in the community. You will link individuals at risk of mental health issues or dementia to the appropriate resources and coordinate care plans under supervision.

The role emphasizes community outreach, collaboration with health services, volunteers and schools, and regular visits to assess and support clients as part of the care team.

Qualifications

• Diploma in Social Service, Psychology or Counselling.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Provide care coordination and service linkage to clients at risk of mental health issues or dementia with appropriate resources and caregiver support.
• Plan and conduct outreach to community and stakeholders to promote mental wellness and awareness.
• Conduct targeted case finding/screening to promote early identification and timely support.
• Organize preventive activities and programmes for at‑risk populations and clients.
• Visit and monitor the well‑being of clients in the community (NTUCH CCMS/CREST).
• Assist in initial assessment of walk-ins and referrals and follow up with intervention plans.
