Job summary

Teladoc Health Asia is seeking a Physician Case Manager to remotely coordinate and manage patient medical care using telemedicine platforms. You will liaise with patients, providers and other stakeholders to ensure effective communication and care coordination throughout the patient journey.

The role involves conducting medical assessments, building personalized care plans, collecting records, and delivering remote advisory services to support high-quality, patient-centered outcomes.

Qualifications

• Medical degree from an accredited university/institution.
• Active medical license with Malaysian Medical Council (MMC).
• Graduate Diploma of Family Medicine or relevant specialty.

Responsibilities

• Manage and follow up cases referred by Teladoc Health.
• Serve as the primary point of contact for patients, healthcare providers and other involved parties.
• Call patients and collect full medical records; understand their medical status and enquiries.
• Conduct comprehensive assessments of patients’ medical conditions, symptoms and treatment history.
• Collaborate with patients to develop personalized care plans, considering their needs and resources.
• Complete clinical case summaries and explain Expert’s Opinion Report to the patient.
• Monitor and evaluate patients’ progress and adjust care plans as needed.
• Cooperate with Medical Director and Care Coordinators in quality and service improvement initiatives.
• Deliver remote advisory services including Expert Medical Opinion, Health Advisory, Chronic Disease Care, Coaching/Education and Referral.
