• Provide patient assessments, plan care, and ensure orders are carried out before shift changes.
• Conduct regular rounds to assess patients, communicate with families, and document care.
• Assist doctors during ward rounds and procedures, following orders and delegating tasks as needed.

General Description

Responsible for rendering professional nursing care to patients within an assigned unit of the hospital. Such nursing care is an integral part of the individualized holistic patient care plan as directed by the medical staff according to the Policies and Procedures of the hospital and in keeping with the generally accepted principles, policies and procedures of the Nursing Services.

Job Responsibilities

1. Understands and carries out nursing care in line with Vision and Mission Statements, Goals and Objectives suited to the scope of Nursing Services.
2. Works shift duties or as assigned by Unit Manager according to Unit’s Operational Hours.
3. Informs Unit Manager when changes occur in circumstances that affect standards of care.
4. Receives and handover report of patient, knows the case in detail, ensures all doctors’ orders are carried out before shift change and on taking over case, and reports any incidents or problems encountered.
5. Visits every patient in the unit together with team members on taking over of duties to assess condition, compare with report, re‑plan nursing care according to assessment or evaluation, determine nursing priorities and plan patient care according to patient’s needs and staff availability.
6. Provides individualized holistic care to patients using the Nursing Process approach with proper nursing documentation in patients’ files, contemporaneous.
7. Reports all abnormal findings and progress of patients to the relevant doctors or consultants.
8. Conducts regular nursing rounds on all patients in the ward to ascertain other additional needs; talks to patient and family members as part of providing emotional support.
9. Follows doctors’ ward rounds and assists them in procedures when required.
10. Carries out orders of doctors or consultants pertaining to care of patients and delegates to other team members according to their assignments.
11. Liaises and discusses with relevant doctors or consultants with regards to care of patients in the unit or ward.
12. Establishes and develops effective communication with all disciplines, patients, and relatives to achieve optimal patient satisfaction.
13. Attends and participates in unit meetings, dialogue sessions and hospital activities.
14. Participates in the health education of patients and their families in the ward.
15. Assists the Unit Manager in the administration of the unit or ward when assigned as shift nurse in charge.
16. Acts on behalf of the Unit Manager in setting standards and expectations in the quality of care.
17. Carries out duties and responsibilities of the Unit Manager in her absence as directed.
18. Ensures charges of equipment or consumable items into patient’s charges accordingly, and checks for completeness.
19. Responsible for checking and documenting supply of sterile packs from CSSS and weekly checking for integrity of packaging.
20. Maintains and documents adequate supply of unit stock.
21. Carries out assignments scheduled by Unit Manager and provides feedback.

Qualifications

• Education: Registered with Nursing Board of Malaysia with a valid license to practice as a Registered Nurse.
• Skills and Competencies:
