Job description

GENERAL DESCRIPTION

Responsible for rendering professional nursing care to patients within an assigned unit of the hospital. Such nursing care is an integral part of the individualized holistic patient care plan as directed by the medical staff according to the Policies and Procedures of the hospital and in keeping with the generally accepted principles, policies and procedures of the Nursing Services.

RESPONSIBILITIES

1. Understand and carry out nursing care in line with Vision and Mission Statements, Goals and Objectives that suit the scope of the Nursing Services.
2. Work shift duties or as assigned by Unit Manager according to Unit’s Operational Hours.
3. Inform Unit Manager when a change occurs in circumstances which affect standards of care.
4. Receive and hand over report of patient, know the case in detail:
   • Progress of the patient.
   • Ensure all doctors’ orders are carried out before shift change and on taking over case.
   • Any incident or problems encountered.
5. Visit every patient in the unit together with the team members on taking over of duties to:
   • Assess condition of patient, compare with report.
   • Re‑plan nursing care according to assessment or evaluation.
   • Determine nursing priorities and plan patient care according to patient’s needs and staff availability.
6. Provide individualized holistic care to patients using the Nursing Process approach with proper nursing documentation in patients’ files and that it is contemporaneous.
7. Report all abnormal findings and progress of patients to the relevant doctors/consultants.
8. Conduct regular nursing rounds on all patients in the ward to ascertain other additional needs; talk to patient/family members as an integral part of providing emotional support.
9. Follow doctors/consultants ward round and assist them in doing procedures when required.
10. Carry out orders of doctors/consultants pertaining to care of patients and delegate to other team members according to their assignments.
11. Liaise and discuss with the relevant doctors/consultants regarding care of patients in the unit/ward.
12. Establish and develop effective communication with all disciplines, patients and relatives, to achieve optimal patient satisfaction.
13. Attend and participate in unit meetings, dialogue sessions and hospital’s activities.
14. Participate in the health education of patients and their families in the ward.
15. Assist the Unit Manager in the administration of the unit/wards when assigned as shift nurse in charge.
16. Act on behalf of the Unit Manager in setting standards and expectations in the quality of care.
17. Carry out duties and responsibilities of the Unit Manager in her absence as directed.
18. Ensure charges of equipment/consumable items into patient’s charges accordingly and check for completeness.
19. Be responsible for checking and documenting supply of sterile packs from CSSS and weekly checking for integrity of packaging.
20. Maintain and document adequate supply of unit stock.
21. Carry out assignments scheduled by Unit Manager and provide feedback.

QUALIFICATIONS

• Registered with Nursing Board of Malaysia with a valid license to practice as a Registered Nurse.
