1) GENERAL DESCRIPTION
The Pharmacy Assistant will be required to perform various tasks within the Pharmacy Services including dispensing, preparation of drugs, compounding, repacking and counseling of customers regarding drug delivery devices to ensure the safe and effective dispensation of medication to the provision of health care.
2) MAIN DUTIES RESPONSIBILITIES
Organization And Management
- To understand and carry out pharmaceutical care in line withVision and mission,Core Values,Goalsandobjectivethatsuit the scope of thePharmacyservicesandthe hospital.
- To understand the serviceorganizationstructure.
- To attend meetings held by the servicesin order todiscuss issues and matterspertaining tothe operation of the service.
- To be the first liner in all dispensing duties which include:
- Screening and order entry of prescriptions/ drug charts.
- Confirmation with doctorsin the event ofillegible handwriting, dosage/ frequency/ durationerroror drug not available.
- Compounding pharmaceutical products for extemporaneous use.
- Packaging,labelling,checkingand dispensing of medications.
- Sending prepared medications to the ward.
- To conduct monthly ward check and pharmacy stock check on slow moving items, short expiryitemand rotate stock ifrequired. Report any discrepancies found toPharmacist.
- To provide patient counseling whenrequired.
- To provide drug information to patients, doctors, nurses, and other hospital personnel. Refer toPharmacistif needed.
- To restock drugs in pharmacy, wards and units and inform Pharmacist In-Charge of unavailability of drugs.
- To ensure adequate inventory level of general supplies,e.g.prescription pads/ drug charts, disposablesand dispensing items for smooth service operation.
- To unpack the ward return medications.
- To prepack bulk medications ifrequired.
- To accurately reconciling andfinalizingdischarge bill.
- To prepare external solutions and disinfectantsfor ward/ clinics;
- Toparticipatein any stock take arranged by the hospital.
- To compile daily and monthly statistics as per qualityobjectives.
- To record and file Psychotropic Drug Prescriptions.
- To ensure precision and accuracy while dispensing andreport to Pharmacist or Pharmacistin-chargeimmediatelyin case of any erroroccurred.
- Keepworkarea tidy and clean.
Human Resource And Development
- Takes personal responsibility for own professional development and keep up to date with professional change and growth by maintaining mandatory learning hours.
- To participate in the orientation of new staff in pharmacy.
- Maintain the record of personal leave and training attended.
Policies And Procedures
- Adheres to the hospital policies, procedure, guidelines and work instructions.
- Adhere to all Acts, Laws, Rules and Regulation governing the pharmacy service at all time. Liaise and coordinate with Doctors and other services on issues related to Pharmacy Services
Facilities And Equipment
- To operateallequipment in the unitaccording to procedureand provides assistance/ guidance to new staff in their use.
- To checkthat all equipmentarefunctioning well and service according to PPM schedule.
- To report toEngineeringpersonnel if any faulty and takes necessary action accordingly.
- Toassistinthemaintenance of equipment by proper cleaning,handlingandutilizationas directed.
- Toassistthepharmacistin keepingaccuraterecords of equipmentand drugs in thepharmacy.
Environmental,Safety And Quality Improvement Activities
- To carry out duties as an employee as stipulated in Occupational Safety and Health Act and Regulations 1994:-
- To take reasonable care at work for the safety of yourself and other persons;
- To co-operate with your employer or any other person in the discharge of any duty;
- To wear or use any protective equipment or clothing provided by the employer.
- To comply with any instruction or measure on occupational safety and health.
- Participates actively in any other new service that is introduced by the services.
- Participates in 5S activities and comply with the specified requirements.
- Practices Standard People Practice (SPP) at all times.
- Ensure compliance of Patient safety goals as safety improvement measures recommended by National Patient Safety Goals.
- Policies on patient and family rights are addressed at all times.
7.Toparticipateactively and encourages compliance to all policies and Procedures of ISO9001:2015, ISO 14001:2015 andOHSAS 18001:2007, HACCP Netherlands, Malaysian Hospital Accreditation Standards, Joint Commission International Accreditation Standards (JCI)and NPSG.
Additional Duties And Responsibilities In The Specific Areas
- To be involved in various committees/ activities assigned by management.
- To be a preceptor to Diploma in Pharmacy Students and to attend any preceptor courses assigned. (if applicable).
- To be a mentor to train and guide new Pharmacy Assistants.
- To perform any other duties as ordered by the Pharmacist/ Pharmacist In-Charge.
3) JOB REQUIREMENT
- Minimum: Diplomain Pharmacy.
- Preferred: Able to communicatefluently and write inEnglish language and Bahasa Malaysia.
- Shoulddemonstrateevidence oftraininginhospitalsettingduring his/herundergraduateDiploma.
- Proficient in common computer applications.
- Ability to check and detect errors during dispensing of medications.
